Terms and Conditions:

I, the individual named above, desire to participate in the events and activities for which this form is being completed, and in consideration for being permitted by Berean Bible Church to participate, I hereby agree to all the terms and conditions set forth in this Participation Agreement which shall be effective on the date of the execution of this form.

I have fully informed myself of all the details concerning the events and activities that I wish to attend through an independent inquiry that has fully satisfied me. I acknowledge that I am voluntarily participating in all events and activities with sufficient knowledge and warning of the danger involved and hereby agree to accept and assume any and all risk of personal injury, property damage, illness, disability, or death, whether caused by the acts or omissions of Berean Bible Church or its affiliates, or their respective representatives, officers, directors, members, deacons, elders, staff, supervisors, employees, consultants, advisors, volunteers, or agents, or any persons acting by, through, under, or in concert with any of them.

I recognize that there are risks involved in participating in this mission trip and hereby assume all risk of injury, harm, damage, disease, or death in connection with my participation in it. I understand and agree that neither Berean Bible Church nor its trustees, officers, directors, employees, agents or representatives may be held liable in any way for any injury, harm, damage, disease, or death that may occur to me as a result of my participation in this mission trip and hereby release Berean Bible Church, its trustees, officers, directors, employees, agents and representatives from any injury, harm, damage, disease, or death, which may occur while I am participating in the mission trip. To the fullest extent permitted by law, I agree to save and hold harmless Berean Bible Church, its trustees, officers, directors, employees, agents and representatives from any claim by myself, my estate, heirs, successors, assigns or other persons arising out of my participation in the mission trip.

I authorize Berean Bible Church through its trustees, officers, directors, employees, agents or representatives to render or obtain such emergency medical care or treatment for me as may be necessary should any injury, harm or accident occur to me while participating in the mission trip.

I understand and acknowledge that Berean Bible Church does not provide health or medical insurance in connection with the mission trip and I agree that I will be financially responsible for any bills incurred as a result of medical treatment, including emergency medical treatment and/or transportation to a medical facility, in connection with my participation in the mission trip.

Safety: Be aware of potential risks and take action to prevent injury. Follow directions from leaders and partner organization representatives. Have a buddy who knows where you are at all times, and don't leave the group or wander off alone.

Respect: Be respectful of sleeping arrangements and provide a quiet environment for others to rest. Respect all facilities used. Follow designated shower times, meal times, lights out, and room assignments. Be considerate and compassionate.
